The Lincoln Healthcare for the Homeless clinic is a Lincoln clinic satellite location at 412 Liberty Street in downtown Durham, adjacent to the Urban Ministries shelter. We serve patients suffering from homelessness as defined by the US Department of Housing and Urban Development (HUD), including clients from Urban Ministries and Durham Rescue Mission shelters, those in family, transitional and recovery housing, people who are sleeping in cars, tents, hotel rooms or unsheltered, those “doubling up” without a lease or those fleeing unsafe situations.
We will provide services for up to one year after the individual has obtained stable housing, and will assist with transition to another provider at that time if necessary.
Walk in appointments are available on a limited first-come, first-served basis

On the campus of Urban Ministries Shelter
We have a multidisciplinary clinic team including physicians and social workers providing case management and counseling for substance use disorders, who are passionate about providing equitable, accessible and compassionate care to people suffering from homelessness. We offer management of general medical problems, preventative care with screening for cancer, sexually transmitted infections, HIV and hepatitis, and vaccinations. We also offer gender-affirming medical therapy and HIV prevention (PrEP). We offer laboratory, radiology, pharmacy, dental and vision services to clients through partnership with the main Lincoln clinic.

- Access to medications, lab work, and diagnostic studies
