Lincoln Community Health Center’s Early Intervention Clinic sees people living with HIV (PLWH) in Durham County. We have an interdisciplinary team targeting at-risk and PLWH in their progression along the HIV Care Continuum. Services are designed to address local gaps and barriers and include comprehensive, culturally competent primary medical care at a site co-located with the Durham County Department of Public Health.
PLWH can have continuity of care from experienced providers and nursing staff, in addition to excellent, timely, integrated mental health, substance use disorder services, and case management services onsite.
