Please arrive 15 minutes early and bring all of your medications with you.
Bring the Required Documents
To stay registered and qualify for discounted care, bring your co-pay and:
Proof of Income
- Recent W2, paystub, or tax forms
- Employer letter with gross income & pay schedule
- Social Security benefit letter
Proof of Identification
- Driver’s license, Medicaid card, or military ID
- Birth certificate or Social Security card
- Work/school ID or immunization record
Proof of Residence
- Utility bill
- Driver’s license or Medicaid card
- Bank statement
- Rent/mortgage receipt
- DMV ID card
Proof of Insurance
- Your insurance card or verification letter
